Select "Reactivate an old account" from the second.

Fill in the name of the deleted account and provide any additional information you deem appropriate.

I am going to guess that you are a Premium member of SL, because if you had canceled a Basic account, you would be gone by now.  Therefore, you have nothing to worry about.  Your Premium account will not be canceled until the end of the period that you have already paid for.  Until then, your cancellation is merely "pending".  You will keep getting your weekly stipend, you may use the Premium sandboxes and receive Premium gifts, and you will be able to use Live Chat and all categories of Support Case.  If you have decided not to cancel the membership after all, contact Live Chat and tell them that you have changed your mind.  That's all there is to it.
