You buy the land with Linden.

The rent is another topic. Renting from another resident is almost always done with paying them Linden, some do take USD over paypal.
If you have a premium account and own more than 512sqm of mainland, you pay your tier directly to Linden Labs in USD.

The key is to remember this part of the definition of L$ in the TOS: "You acknowledge that Linden dollars are not real currency or any type of financial instrument and are not redeemable for any sum of money from Linden Lab at any time."  In other words, don't expect Linden Lab to accept L$ in payment for land or any other bills.  You'll have to sell your L$ on the Lindex to other SL residents and then give LL real money to pay your bills.  It sounds like a picky technicality, but L$ are just Monopoly money that we use in SL.  LL acts only as a currency exchange, managing the flow of USD and L$ as we buy and sell L$ to each other.

It depends somewhat on the land. Small parcels on the mainland are always bought with Lindens, but you pay tier every month in US dollars. You must have a premium account. Some small mainland parcels can be bought with Lindens on the Linden Lab's own auction page. 21 mainland parcels are for sale right now on that page, from 512 m to 12,800 m. Reach it from your avatar's Dashboard website. Other mainland parcels can be bought in-world from private sellers, by clicking the "Buy Land" button in the About Land dialog window while you are standing on the land. You are paying in Lindens, so you must have enough Lindens in your avatar account to cover the cost or auction bid. Your tier will automatically go up to cover the size of the land once you take ownership, if it is not already high enough.

I believe very large parcels, and private islands, must be bought with US dollars.

This is different from renting land. When you rent land, your name does not appear in the About Land box as the owner. You must pay rent to the real owner (landlord) every week, normally in Lindens.
