This is an old race condition (between server messages and the viewer renderer) that got introduced together with the ”interest list” changes, years ago...

There are workarounds for it, but not all viewers got them. The Cool VL Viewer provides ”Refresh visibility of objects” in its ”Advanced” -> ”Rendering” menu (shortcut is ALT SHIFT R), and auto-triggers this workaround on login, far TPs and region borders crossing, meaning you rarely ever need to trigger it manually...

This problem isn't unique to SL, I've seen it in several other grids, which seems to confirm it's viewer behaviour and not server-side. 

One oddity, (which Henri might have explained above), is that right-clicking on a missing item not only restores it to view, but all the other hidden items come back as well.

This is a race condition between the servers and the viewer. The Interest list changes affected both (and the object cache was fully rewritten viewer side), meaning it is not surprising at all that you see the problem occurring out of SL (not to mention that OpenSim servers reproduce the same messaging as SL servers, even if with different algorithms).

Not all objects, but all objects in the same spatial partition (objects are grouped in the renderer, thus why they get affected by this bug as a group).
