I unpacked an item but it's just a box on my right hand. How do I fix this?

That's because you are wearing the box.  Once you have emptied it, you no longer need the box.  The contents were transferred to your inventory. Open the Recent tab in Inventory to see them.

it is possible a item is double boxed.

But are you sure you bought a skin?... or is it a applier you attaced to yourself.. ?

The last one will only work if you have a compatible body.
