I'm not sure what you mean by "permanent."  Nothing is permanent in SL.  And if you are talking about land, anywhere, the answer is always that there is a monthly fee.  If you own a private region, your standard rate is $295/month.  If you are a registered educational or non-profit organization (501©3), you can get a discounted rate, but that requires a bit of paperwork.

You may buy a new island direct from Linden Lab.  That will mean paying a hefty one-time setup fee.  Or you can buy a region from another SL resident, for example one that you see advertized in the Land forums.  If you do that, you pay that resident the purchase price you agree on, and you pay Linden Lab a $100 transfer fee.

If you are not buying an entire region, the purchase price (if any) and monthly fee is between you and the region's owner.  Essentially, you are paying that person rent, and that person is paying Linden Lab the monthly fee for the entire region.
