That's right; you can't. On the default female avatar mesh, the chest never shrinks completely. One tip I've heard is to use less bouyancy for the breasts, as it makes them appear flatter and smaller, but they'll never go away completely. Skilled child clothing creators use techniques to further minimize/hide this.

You can use a male shape, but the shoulders and spine of the male avatar mesh get very, very odd looking the smaller they get. (So I have heard.)

You can try looking for a mesh child avatar. If you want your own shape uniquie (ish) shape, though, try looking for good child's clothing, and tweak, tweak, tweak your shape. Then tweak some more.

Added to other posts, make sure its a child skin you use too.  Shrunken shapes do not make a child avatar. Breast shadows on skins will always look wrong.
