What is the procedure if a private region tier is not paid?

What happens if a private region falls behind on payment? For example, is there a grace period? Does LL reclaim the region? I can't find any referrences in the knowledge base.

after a short time the region will taken offline, if payment still stays out the sim gets offline definitive, all what's been on the sim, and isn't reclaimed by renters or others on the sim,  gets deleted.

the coordinates are free for other sims again.

No, there is no grace period, except by accident.  A group I work with has abandoned several regions over the years.  One of them hung around for over a month after the last fees were paid but the others vanished within a day or two.  One was gone a few minutes after midnight on its final day.

Regions take server resources.  If you can't pay for them, Linden Lab will find another resident who can, as soon as they can.  Having said that, though, the region contents are in fact held off line for a short, indefinite period of time, just in case residents need to get back in to retrieve things they left behind. "Short" usually means  a week or two.
