How would I make an accessory to go on an avatars chest?

basic building really. Unless you want to make it mesh or sculpted prim.

You can try the Ivory Tower of Prims to get ideas on what you can do with prims. Builders brewery is a good group to join for advice on building.

So long as you just want to attach it to your av's chest, rather than trying to link it to a rigged prim blouse or dress (which won't work), then it's just a matter of selecting the object in inventory, right clicking, and choosing Attach >> Chest.  Thereafter, whenever you select Wear or Add from your inventory, the object will attach to the spot you defined.

Where on the chest would it go? If it doesn't go in a place I want it, can it be moved?

I'm still fairly new to the game in tems of I don't know the tems for most things...What I'd like to make is this:

Sure.  You move and rotate it the same way that you move and rotate anything else.  Use your basic Build/Edit tool.  See http://community.secondlife.com/t5/English-Knowledge-Base/Build-Tools/ta-p/700039 and then visit the Ivory Tower of Prims in world to do tutorial exercises and practice until you know what you are doing.  Or consider taking a beginning class at Builder's Brewery or another educational site.
