Why does Second Life log me off every time I teleport?

Most probably you are wearing too many scripted objects. Try to detach objects you are wearing and check if it works then to find out what exactly is causing your problems.

You might also be trying to teleport into regions that are overcrowded (like nightclubs or shopping centers), try searching your map for uncrowded regions and see if it still happens.

Teleports most often end in a crash due to either lag or wearing to many prim heavy or scripted items.

I had the same problem, so clear cache, then...

1. Open Develop (control + alt + Q) then RENDER TESTS - FRAME TEST 

2. Open Develop again, UI - DUMP SELECTMGR

3.  Develop again, then UI - DUMP INVENTORY
