how do i make a avatar with a last name

As for all recently created accounts, you last name is Resident. However, you can use a display name (which you can change once a week). You cannot do that from your dashboard, you have to create your display name inworld (under the "me" menu) or on web-based profile: https://my.secondlife.com/settings/profile

Make sure you don't use weird typos such as $%;**$. Use regular letters.

You can't have any spaces in the name of a new avatar. They have to be one continuous word. 

You can make a AngelUnknown (if the name is not already taken), but you cannot make an Angel Unknown.

You can, as Val said, use a display name, so you could register a new avatar as AngelUnknown (for example) and then make his or her display name Angel Unknown.

Although your last name will be "Resident" (unless you have signed up under the jurisdiction of a large company) you might like to try adding your preferred last name into your first name and show the distinction using capitals. For example ..... JohnSmith Resident ".............. Other than this, use the "DisplayName" feature, mentioned by others.
