I'm a little unsure what you mean.  You can DOWNload things from Second Life to your computer, or you can UPload things from your computer to Second Life.  But if you have a RL photo, it can't have started in SL, so.....

Are you asking about how to UPload a RL photo to SL, or are you asking about DOWNloading a SL photo to your computer?

To UPload, open Build >>> Upload ... Image.

To DOWNload, Find the image in your inventory, double click on it to open it, and then click the Save ... button in the lower right corner.  There won't be a button unless you have full perms for it.

A small addition to Rolig's fine answer...

Befor you upload the photo, open it in Photoshop or a similar image editing program.  Re-size and/or crop the image so that its dimensions are a power of two...for example, 512 x 512 pixels, or 1024 x 512.  Your maximum dimension should be no larger than 1024 pixels.
