Unless you have done something extremely unusual, all you have is bake fail, which is typically easy to cure.  See http://wiki.phoenixviewer.com/fs_bake_fail

Three are 4 things every avatar must have that cannot be removed.  Shape, skin, hair base and eyes.  If any one of those cannot be loaded for any reason, you get the cloud.

 

When the asset servers are on the fritz this happens because SL basically times out waiting for the pieces to download from the asset server.  However (knock on wood) the servers have been stable of late.

So .. the quickest option is to remove all items then go into inventory and replace your shape, skin, hair and eyes.  You could also do this by choosing a default classic avatar from the list.  After that, put your original pieces back on one by one until you find the problem item.
