Exactly how are you going about changing your clothes?

If you click "Add", the clothing layer or attachment will simply be added to whatever you're currently wearing.

If you click "Wear", the item will displace the current clothing layer or the attachment on that attach point.

If you click "Replace Current Outfit", the thing(s) you've selected will be worn, but ALL your current clothes and attachments, save shape, skin, eyes, and hair base, will be removed.  Only use this option if you're sure what you are selecting is the complete outfit you want.

Sometimes, even if you've removed an item, it will appear to return briefly.  I usually see this with system skirts, though, not with attachments.

Did you make this part of an Outfit in Appearance?  That'll do it every time.  Are they part of an outfit?  You may have to wear tthe outfit first and then take off and replace them with something else.

Finally, when was the last time you cleared your Cache?  Be prepared for a LONG log in afterwards, but many linked items will no longer be linked (like outfits).

Clear Visible Cache first:  that is a script you use to clean up visible things.

Clear Cache: The big time fix up.  Be sure of what and where that cache is and where its located.  Clearing your Cache will remove EVERYTHING in the cache file, so if you have other stuff in the file, move it first.

Those two bits of clearing should fix your problems.  Just remember Clear Cache = Long load afterwards.

@Damien.

Why would linked items like outfits no longer be linked on clearing cache? The cache is merely a local folder on your computer and soon as you clear it and log in, your inventory including linked outfits will be downloaded again from the server. I've never heard of anyone having an issue with linked items becoming unlinked after clearing cache.

To avoid a longer log in while the viewer is clearing cache, just delete the cache folder in AppData instead of clicking the clear cache button. Unless you have a huge inventory, it  should reload in a few minutes after logging in especially if you log in to a low lag Linden water sim like Smith.

Edit: Regarding 'clear visible cache' scripts to try and force the sim to update nearby visible objects, I believe you can achieve the same effect by changing groups.
