You need commercial land for any kind of "business." If you intend to make money by offering a service, that's a business, and you'll need commercial land for it. Doesn't matter what land. I suggest M-rated, because chances are if all goes well you'll have naked women about sometimes and in "awkward" positions while "giving birth." I don't suggest a shopping center, but perhaps some kind of community where people can walk around and find you and some other, related businesses. These places are around, but sometimes a little hard to find. You can also set up your clinic somewhere completely random and unrelated, and advertise well.

Note: The reason I don't suggest a shopping center is that most are fairly open, and the shops fairly small. At least, the kind of shopping center I'm thinking of.
