Please remove the person's name from your post so that you don't get in trouble for violating TOS here.

How did he find you? Unless you closed your account and are using an alt that absolutely nobody knows is you, a determined stalker could probably find you, given enough time.  Just as in RL, we tend to hang around the same people and places in SL most of the time. Unless you ditched your entire wardrobe, maybe he recognizes a favorite dress. If even one person knows who you are, it wouldn't take more than an innocent slip of the toungue to reveal your identity.  Also, it's hard to hide your style of writing in chat, your common typo patterns and your abbreviations.

If you have any reason to believe that he knows your RL identity and may be stalking you there, don't hesitate to let the local police know, BTW.  Stalking is a serious crime, and can lead to even worse.

Something might be attached to your avatar. There are many ways.

If you want it to stop, you could try:

login to SL, go to busy mode, create a new Home location, check everything you are wearing, clear your cache, relog and see if your still having the problem.

If you do, you could use an alternate Viewer.

I don't know a lot about the current combat/communications equipment in SL, maybe another Resident here does.

Edit: As mentioned, you should use Options to edit your post and remove the name.

"Following", in this context, doesn't mean that the person is following you physically. It means that the person has your profile on his RSS feed, like seeing when people make changes to your facebook page. I find the whole concept of facebook pages and the loss of privacy more than a little creepy. Yes, it's possible that this is one way for the OP's stalker to be "following" her.
