That generally means that you have an unreliable Internet connection, so the servers cannot complete the login process.  Until login is complete, your avatar appearance files will not be downloaded properly to your viewer, so you will appear as a fluffy cloud and you will sometimes get that message that your clothing file cannot be found.  The solution is to repair your connection.  First of all, do not use a wireless connection to the Internet.  Many people have no problem with wifi, but Linden Lab does not support or recommend it. The same is true for satellite connections.  Then, reboot your modem and your router by unplugging both of them from the power for a few minutes.  Plug them back in and restart everything.  That will usually improve the reliability of your connnection.
