An applier is an attachment or HUD used to "apply" a texture to a prim or mesh object attached to the same avatar. Usually, it's for prim breasts, although I know that at least one or two creators have made skin appliers for mesh hands. Appliers are usually used to put a) a skin texture onto prim (or mesh) breasts so that they match the skintone of the user; or b) apply a clothing texture to prim (or mesh) breasts so that breasts appear to be covered by the same top that covers the rest of the avatar - same color, pattern, neckline, etc. as the system breasts in the same top.

If you don't use prim or mesh breasts, you can ignore appliers. They won't be important to you. If you DO use prim breasts, appliers are the only way to get matching tops to cover your breasts.
