Buy a nude skin.  TzalBare has some great skins in a variety of colors with some great features.  Her skins usually come with an avatar shape as well, but you do not have to use the shape.  It is nice that the eyebrows, make up, cleavage, etc... are all included with different options.

They don't come off. The underwear on the default avatar is painted directly onto the skin, and is not removable. You'll need to shop for a new skin. You can find free ones, some of which are very good and available as free gifts for joining certain groups. These skins will also make your avatar look better and less "noob-like," so it's a win-win.
