You don't need a premium account to rent anywhere in SL.  You will pay rent to a landlord every week or month depending on the terms of your lease.  The Landlord will use part of your rent to pay the tier. 

Rental rates are determined by the landlord.  They will be at least the amount of the tier plus a profit.  Rent is not regulated and a landlord can charge what they want, and change the amount on the next due date if they choose to.  However mostly rents are stable and are not raised unless tier rates go up or when a new tenant moves in, unless your initial rental rate was a promo rate that expires.

Just to clarify a little...

Although we use the term loosely to refer to any land fee, strictly speaking "tier" only applies to Mainland that you own (not rent).  In fact, according to LL, "tier" is not a fee at all, but a capacity to own land.  If you are at the 2048 tier level, you can own up to that amount of land.  But, you only pay a land fee on the land you do own.  If all you own is 1024 sq. m, then you only pay $5 USD per month (the fee for 512 sq m, plus the free 512 you get with your Premium membership.)

Another aspect of this is that you must be a Premium member before you ever have to start worrying about tier, because only Premium members can own Mainland.

If you "buy" a land parcel on a private estate or rent land on the mainland, you pay a weekly or monthly fee to the land owner, usually in $L.  This is a transaction between you and the land owner, and has no effect on any land fee that you pay to LL.
