You don't have to be  a Premium member to cash out, no. I've never been Premium, and I run several businesses in SL and cash out frequently to Pay Pal.

Being a "verified" pay Pal user means that over on the Pay Pal site, you have associated your PayPal account with a bank account. You'll need that anyway as a final step you get the transferred funds back out of Pay Pal and into your bank account as real money.

Every time you cash out, you have to manually enter the e-mail address for the PayPal account you are sending the money to. It does NOT use your "Payment info on file" payment method, even if you have also specified PayPal as your payment method there, and can't automatically default to the PayPal info that you entered for payment info on file, if that was what you used. So you don't have to use PayPal as your preferred payment method to be able to cash out to PayPal, either. In fact, I don't think you need payment info on file at all.
