Hand's position out of line with where they should based on animations

Question

So weird problem, on many animations, poses and item generated poses (items being held etc) my hands will position too far apart or in the case of single hand poses, they will position offset from where they should be.

 

Best example is this pic. I am thinking it might be a firestorm issue because shape, attachments etc all seem to have no effect on it. 

 

Anyone have any idea what is causing this?

That is usually caused by a too wide (or at least much wider)shoulder width than the creator of this pose used! Try making your shoulders less wide and sometimes even your arms a little longer or hands bigger sometimes helps too (especially for people running around in 0 hands) x3

I have not had this problem, exactly, but I have had several times when my add-on SLink hands or feet did not fit on my body correctly, even though they had fitted perfectly the previous session and I had changed nothing.

A re-log generally fixed the problem.

This has persisted through several wildly different shapes with Lara, Legacy and Reborn all with standard included hands, closest thing to solving it I have found was a shoulder morph that bring them shoulders in without changing the upper body shape . problem with that is, it can sometimes look like I am shrugging my shoulders with certain poses
