How do i create my own custom tattoo?

I've been trying to have a custom tattoo done for months and have gotten no where with it. I would like to try my hand at making my own. Is it very complicated?

I've never created anything before, so I would need to know all the basics. I have photoshop however my knowledge of it is very limited and i mean limited.... but i'm willing to see what happens. 

Any advice would be greatly appreciated!

If you have a version of Photoshop that does 3D, this may be of use...

You said your knowledge of PS is limited, so I expect you'll have better luck avoiding that 3D method.

ETA: I doubt any of those tutorials mention "local textures", which are very neat and will save you both time and money. Rolig Loon took the time to explain them here...
