I have not changed anything and I cannot see anything that would violate any standards. so SL is broken and I have not idea what to do

I have noticed that there are issues with the AO and my avatar that result in things like her hands sinking into her buttocks or a hand and part arm going through a breast. Is this the AO or the avatar problem? What is the solution?
