Mesh clothing (which is most of the clothing sold in SL these days) usually consists of two parts.  There is the mesh object itself...the dress or top or whatever.  Then there is an "alpha mask".  This will have the inventory symbol of a shirt with a grid pattern on it.  It is a "clothing layer", painted directly on your avatar body, and therefore is not affected by the Detach command.  The purpose of the alpha mask is to make the parts of your body that are under the clothing invisible.  If this isn't done, your body can poke through the mesh clothes, making it look like moths have been busy.

When you wear mesh clothes, you should right click the clothing object and select ADD.  Then do the same with the alpha mask item.  When you take off the clothing, right click the item (either on your avatar or in your inventory) and choose Detach...and ALSO right click the alpha mask layer in your inventory and select Take Off or Remove from Current Outfit.

The above applies to mesh clothing designed to work with "Classic" avatars.  If you are wearing a mesh body, the body itself will have a separate alpha mask layer that hides all of your classic body.  Then you will use a set of HUD controls to mask out the parts of your mesh body that are underneath whatever clothing you happen to be wearing.
