i was having this issue and did a little investigating. if you're on firestorm, it turns out there is a soft limit of 54 bom layers. (though the actual limit is 80-something. but after 54, things go wonky.) you can still wear the layers, but they are not shown as worn in inventory, so they cannot be edited unless you remove something else. you can still remove the layer by going to the worn tab in appearances. not sure if that was your problem, but i thought it was worth mentioning in case anyone else encounters it.
