This used to happen to me all the time when i was using a slow dialup phone connection. After switching to broadband, the problem went away. Teleporting to certain regions and at certain times seemed to work better than others. I learned by trial and error that i could always teleport within a region without ever getting logged off. So I would log in by typing the name of the region i wanted to go to, and then teleport to my landmark if it wasn't easier to walk or fly there. Good luck!
