Why do my attachments appear in my lap instead of where they should be?

Why are my attachments appearing in my lap (or up my butt) when I log in? Also, they appear that way to others when they log in or after I teleport but appear fine to me.

I'm having a flashback to 2008 when it was quite commonplace to find a shoe or hair or other attachment up one's butt or tucked behind a knee after teleporting or logging in.

It might be an optical illusion.  What happens when you or people around you change group tags? Or do you have to detach and re-attach the attachments when this occurs?
