Mesh uploads as twisted garbage (search didn't answer)

I've been trying to upload any weighted mesh for some time now, and nothing works. I have boiled the problem down to the barest elements, and nothing works.

Taking the standard Linden Labs skeleton, putting it into 3DS Max, and then exporting it as a DAE gives me this.

Absolutely nothing was done to any part of the model except removin the non-mBones from the DAE file, since the file wouldn't upload with them.

Somebody please tell me what is going on. I searched for an answer here and found nothing. I've talked to people in world and gotten no answers. Every change I make, different skeleton I use, or different appraoch I take just gives me similar twisted up garbage.

This is how it works for me with 3ds Max 2015:

1) I have set my System Units, Display Units to Centimeters.
2) Downloaded the Skeleton from here: http://wiki.secondlife.com/wiki/Mesh/Rigging_Fitted_Mesh#Download_links
3) Imported the Skeleton.Male.dae file with File units converted to: Centimeters, on import.
4) Selected the BaseMesh > Export Selected with Scene units converted to: Meters. UpAxis: Z-Up.
5) Pasted the entire mBones list after the bones which are in the dae file already. (doesn't matter if it will have duplicate bones)
