The same way that you would anywhere else.

1. Visit interesing places where you're likely to find people with a common interest

2. Join groups that do things you are interested in.

3. Introduce yourself and start talking (typing).

In SL, you have a couple of additional advantages.  Everyone has a name over her/his head, so you don't have to worry about remembering names or having other people remember yours.  Also, everyone has a personal profile.  You can read anyone's profile by right clicking on the person and selecting "Profile".  You'll be able to see whatever they decided to make public -- instant conversation starters.  Be sure that you fill in your own profile to make it easy for other people to see what your interests are too.

You might also try posting in the Make Friends forum.  There are always people there looking for a friend.

You do NOT get them by simply sending Friends requests to everyone you pass by, that's for sure.  Most experienced SL residents consider getting an unsolicited Friends request to be extremely rude.

Walk up to people, say hello in local chat.  Because local chat can be heard (read) by everyone within 20 meters, use their name, so they know that you are saying hello to THEM.  Don't just say, "hey, sup?"  Say, "Hello, Lindal.  I really like that hat you're wearing," or some other opening comment.  After talking for a while in local chat, you can switch to IM for more privacy.

After talking to them for a while, if you like them (and if it seems that they like you) you can ASK them, "May I add you as a friend?"  If they say yes, THEN send them a Friends request.

Once you have made a friend, make an effort to keep them.  Call them now and then when you see they are on line.  Invite them over for a chat.  Let them know about the great sale you found.

You could go to clubs. There are many many clubs in SL. Be a regular in a few that interests you. Soon, you will see you have friends. Best to start with local chat .. when you are conversing with a stranger for the first time. A greet is usually the right way to start, saying, "Hi, How are you?" Talk of music, outfit, places etc. There is so much to do in SL. Start exploring through the Destination Guide.
