I'm still having this problem - accompanied with a message on the screen saying... We are sorry, Linden Lab has discovered degraded performance on your connection to the region you are on. You will need to restart Second Life and log into a new region for the next 30 minutes to an hour. We apologize for the inconvenience. It seemed to go away for a bit but then came back. It only occurs in the Firestorm Viewer - latest version. Followed all the advice on the Phoenix site, and opened a Jira there. The only way I could fix my avatar was to tp to an empty sim, took off all my huds, done the Reset Default Female Avatar and low and behold I could rezz. I then changed back to my main avatar and saved it without any of my huds attached. Logged out and then logged back in and I was a cloud again and had to repeat the whole process every time I logged in. It's not my connection for sure, its hard wired and I am on 30mbs and I am using an Nvidia GTX 560 ti card and have 16gb DDR3 Ram usining an Intel Core i7 processor. I just gave up in the end and went back to using the old Phoenix Viewer instead which works every time. Shame really as I like the Firestorm viewer but it just does not like me. Maxi

I have 3 viewers installed, the official one, the old Phoenix one and the latest Firestorm viewer. Were you getting the following message when logging in using firestorm... For me I was getting this message for over 2 weeks everytime I logged in. I was just a cloud, tried everything like you said. Gave up after 2 weeks. I then tried again after 2 weeks then magically I could rezz my avatar again. I didn't need to change anything, it just suddenly started working again. I'm so annoyed with the whole experience I decided to take time out because logging in is not a fun experience anymore. I'm not even sure if my avatar will rezz again and I do not want to spend my time looking around for empty sims to rezz my avatar, delete cache, gestures, detach/change hair base ect... Another thing I noticed is if you had previously rezzed some items using an old viewer like Phoenix then try deleting them with a newer viewer like Firestorm it will not let you ( for me anyway ). I had to go back to the viewer I rezzed the items with to even see them and delete them with that particular viewer. The problems are becoming so complicated. I have been in touch with the Phoenix/Firestorm people on their Jira and they are also confused about the "degraded peformance" message I put at the top of my message here, LL are not supplying any info as to what it means though so they are also confused without getting any answers from them. Changing start up land plots from 512 to 1024 is defo needed, even if you keep the same prim allocation rezzing stuff there. Everyone is now using scuplties and mesh. I joined SL in 2007 before the sculptys and mesh and have my own mini plot of land that is FREE there and I pay for using my account that is registered with my bank details... but I cannot really do anything with it except rezz prims, individual sculptys and mesh. I cannot really make anything more there in this tiny space. I ended up going the Standalone Diva Distro sim thingy which lets you host your own sim on your computer. Importing the content i made on my standalone sim hosted on my own computer is what I now do. I do not have to pay anything for hosting as it is all on my own computer.
