Often when a teleport fails, it's because we're wearing something scripted that is preventing us. Try detaching anything that may have scripts in (ie hair, boots, genitalia, HUDs) and try teleporting again.

It can also be because a region/sim is being restarted.  Have you tried to teleport to other regions?

There's a nice selection of suggestions (including a good video) at https://wiki.secondlife.com/wiki/Troubleshooting_teleporting .  Also, read the other excellent disussion at http://wiki.phoenixviewer.com/doku.php?id=fs_tp_fail .  As Marigold says, wearing too many scripted items is one common reason for teleport problems.  Another is an overly restrictive firewall.  Take a good look at both of those possibilities.

Jewlery, hair, and shoes can have an enourmous number of scripts. I've seen avatars wearing over 800 scripts. The more scripts you have on, the mode difficult it is to TP. The servers have to do way more work and a busy region may fail.

In the market place are script counters. They are free and often look like a weight scale. They will tell you how many scripts you are wearing and the amount of memory they are using. Less is better.

Well made jewlery, hair, and shoes have an option to remove scripts. Make a copy of the item before removing the scripts. Have a scripted copy and a de-scripted copy. Wear the descripted copy. When I'm not in a combat sim I typically wear between 3 and 8 scripts and I'm into nice hair, shoes, and jewlery.
