how do i get an item back when it fails to rez?

I believe you may have to contact SL Support by filing a Ticket.

Support may request that you follow the suggestions at the link below.

If the issue is on SL's side, a Linden will attempt to repair folders.

You may also attempt to contact the seller or provider of the item and request a replacement. They are not obliged to do so but they might help.

If you have Premium membership you can use Live Chat or a Support Ticket. If you have Basic, unfortunately there is no support for either items rezzed inworld or items in Inventory.

Have you checked your Lost and Found folder for the item just in case it was returned there?

Have you cleared your cache from Me > Preferences > Clear Cache. Click button then close and reopen the viewer and log in.

Failing any remedy working, I would suggest you contact the creator of the item, explain what happened and ask for a replacement. You may get lucky. :)
