This is a known bug ( https://jira.secondlife.com/browse/VWR-24137 ), apparently related to failure of Voice to close properly when you exit the SL viewer.  According to the most recent note in that JIRA, you can resolve it by " opening task manger and ending SL voice * 32 process."

Consider visiting the JIRA and adding your own comments if you feel moved.

uninstall the viewer & delete all hidden files... then reinstall the viewer < the new version..simple!
