Log into the second life website.  Go to land manager on the left side and choose Linden Home.  The page will include the SLurl to your Linden home.  Clicking the link will either open the viewer if you're not logged in or bring up the landmark in the viewer if you ARE logged inworld.

ETA...it might actually say, Go To Linden Home.   I don't have one currently so best guess.  It IS there though.
