Looks like the center of a mega-prim is below 400 meters.  Of course it can be multiple objects of various sizes instead of just one.  The map shows the "average color" stored in JPEG2000 texture metadata when rendering them on the map.  If this data is missing from the texture you get "50% grey", same as a missing texture in-world.

It's likely this - I moaned at my landlords for weeks about this (thankfully they're friends!) and even submitted a ticket about it...

To my mind it made our sim look "broken" so folks didn't visit via map browsing.

Turns out that a lot of sim surrounds use mega prims and the bounding boxes confuses the map rendering routines (also beware - there's a very nice planetary system that leaves circles on the map too) - eventually we found a lovely 3/4 sim surround that works & doesn't obscure the map and for our part of the sim we use prim/sculpty off sim elements.
