I forgot my email and password and only know my username

as you are currently logged into your browser i will assume that you have that on automatic login 
then you could simply go to this page  to change your email settings 

if you are speaking about another account you could try this page and then only fill in user name - and send instructions, however you woul dneed to be lucky enough to have used a email account that will notify you of reiceived emails 

 

if that all fails, then you could try to send a ticket to linden labs using the account you are on atm
