The other thread by this name proclaims a scam. I'm not saying your enterprise is the same, but that and the application sure seem... unusual. As for the application, why would you want that information and why would you want it filled-in offline, on a web site, where you can gather RL information through cookies and other methods?

Unorthodox methods = bright red flags.

Just saying.

Not to mention it states on the application that the modeling position is only for “exposure” with zero guarantee of payment. If I’m going to model for someone I want to know I’d be paid for my time.

Meh, the questionnaire isn’t asking for your social or anything. It looks to me like very basic questions. To me, it’s more professional than someone saying “your avi is beautiful, wanna do a porno? Lol” It helps to get an idea of time and an idea of their expectations.

Also, SL porn usually doesn’t pay models anything because...well...there isn’t really a market for it. Not one that pays anything anyway.

Unusual to you. That doesn't mean, that it is actually unusual.
She uses Google docs. Absoluttly legit and no problem at all. Thousands of people use this service for exactly this.
(I might be biased though, as I used to work for Google)

Yeah, I wouldn't be too worried about privacy of a Google Doc form. On the other hand, once you click through there's this:

Hi Qie,

most porn producers do it for fun or to support another business. Promoting escorts is an obvious one (That is what we do at SLA Media as well).
I got to know one,who has a porn "business" and group advertising her lingerie & fashion shop. She is making pretty good money with this.

Oh I can definitely believe that. This particular ad, though, makes me skeptical, charging "for portfolio and headshot work" which seems the barest minimum role an agency would assume.

Back ages ago there were some common scams, not particularly associated with adult content but "modelling" in general, which were really simple tricks to part newbies from their money on the premise that they earn L$s if only they first got these photographs they needed to pay for, wearing these clothes they needed to buy. Maybe this isn't the same scam, but I'm not seeing much to suggest it's different.

that might well be the case.

It`s kinda odd to charge for promotional photowork. We have even our own studio for that and off course our girls don't pay us a single L$ for us making those shots or for them using our premises to take them shots themselves.

Actually most professional models / escorts come with their own selection of sexy pics done beforehand. It aint that difficult after all.
