as Selene says it was a Teleport attachment bug back in the day. What would happen is that the attachment points would not transfer properly. When so then the attachments would default attach in the client view to the pelvis attachment point

to get "ruthed" was also something that would happen.  Ruth is the name given to the first female avatar.  When a avatar is first rezzed it is the default shape (female), then the slider values are applied.  On teleport (and sometimes login) the slider values and options would not get applied. The avatar (including male avatars) would remain displayed as the default female shape (Ruth) hence the term. To be ruthed

Linden have since fixed all of these issues pretty much

a thing with being ruthed is that it is/was a client side issue.  Sometimes we would be ok on our screen, yet not on other peoples. which was a total pain

a similar kinda thing used to happen before the avatar layer baking server side service.  The baking used to be done on our viewer then uploaded to the server.  We would bake ok on our screen, but the bake would not always download to another person's viewer, so end up naked on their screen

now that we have mesh, is lots of deja vu in the effects we see on our screen. Bits all over the screen, some people's avatars never resolve and like in the old days, we pretty much just be polite and don't say anything in public chat about it.  Just send a discreet IM after a while if a person never resolves for us

Hair and sometimes shoes attached to the rear was a very common TP bug for a while.  I encountered it a lot in my 2007 days, but can't really remember when it went away -- and apparently never saved any pics of it.

Around that same time, I used to encounter all sorts of borked avatar positioning when "standing" from a sit or dance animation.  Some of my better ones ones:

That's proper Exorcist stuff, that is. Including the deleted "spiderwalk" scene.

I remember ruthing. It happened to my Dom once just as he was TPing in with menacing intentions. I laughed so hard.

The biggest thing I miss about Ruth avatars was that when men TP's into a place, they started out as a female Ruth before the male features kicked in and when they (those men) were told this they would absolutely freak-out and go APE-SH*T-BONKERS. I loved watching those reactions most of all; major entertainment.

i totally forgot about the  head backwards thing, and the bend over backwards. Pretty funny 😺 lol

I kinda love that the prototype for the SL avatar is a woman.

I like to think that male avatars were a sort of afterthought: Ruth needed someone to dance with, so the Lord Rosedale took a handful of pixels from her side and created Man, to be our helpmate and fetch us things from the kitchen when our feet hurt.

This still happens today at times.

Especially if someone is dealing with a slow connection and/or a really busy place.  With mesh bodies, others can see you in pieces even when you look okay to yourself.  Common for many of us to see it when we enter a busy region, but it can last for quite a while for some folks.  I'll even see myself all broken up sometimes if I log in to a busy place.  This is a picture I grabbed of myself when I re-logged after a disconnect, to a super busy region.  It took long enough to sort me out that I was able to snap a picture.
