Years ago, there was a journalist that had a female avatar in SL. She went around interviewing people, and had a blog where she reported the interviews. There were also some videos or graphic novels online in one of which she came out of the monitor into RL.

Does anyone remember her name?

Holly Would.  Film was "Cool World", character was voiced and played by Kim Basinger.  Although Holly wasn't a journalist...she was a nightclub singer.  The male lead was her cartoonist/lover/fall guy, played by Brad Pitt.  So maybe we are remembering two different things.

I think Max was the other way around, started out human, was in an accident, then became a digital entity.

Yes.  Max was the digital re-creation of ace reporter Edison Carter.  In the original movie, Carter was killed, and a young computer nerd simulated him digitally in an effort to recover his memories, to solve his murder.  In the TV series, he recovered from his injuries, but Max, the digital simulation of him, continued on in a sort of partnership with Carter and his pals.  I LOVE MAX HEADROOM!!!

I liked the Ronald Reagan version, too. In the Doonesbury comic strip.
