How can I get an old account back after forgetting the password and the email I used

I made an account about 5 years ago and about 3 years ago I was kicked off and wasn't able to log back in for some odd reason. I want to try and log back into it but I don't remember the password or the Email I used to create it... Help?

• Fill in your current email address.
• Issue type: Account issue
• Account Issue: Lost Password
• Fill in the name of the account that has the lost password.
• Under " Please describe the issue" give the full details of your problem.

If you can be helped you will receive an email.  Follow the directions from that email and supply the information requested to the best of your knowledge.
