When you join Second Life, you choose a basic male or female avatar from a number of choices.  Once you enter Second Life, you can modify the avatar by using the appearance editor.  You can read about it here:

There are places where you can get freebie clothing, skin, hair, etc.  Type 'freebies' into the search bar inworld or in Marketplace to find freebies.  If you wish to purchase items, you can buy Lindens off the Linden Exchange on your Dashboard or try to earn Lindens inworld.  The fastest way is to buy Lindens.
