The only option would be to have the rooms on seperate parcels and turn off the option to have avatars visible from outside the parcel and then you could also install a security tool to eject everyone from that parcel...but you probably won't archieve a level of privacy one would have in a real life clinic, that thing simply doesn't exist in SL.

Script wise it is trivial to look for a porson's position in the sim and if "inside a room" send them a RP style message or even teleport them home. If you use experiences you can also track the location on their camera and give them a "please don't peek through the curtains" warning.

Do you know a scripter? Ask them for help. If not, ask in the wanted area of the forums.

What you want is a dividing wall with a door in it.

Next to that door you place a roleplay bouncer to match the roleplay clinic with its roleplay professionals with roleplayed qualifications servicing roleplay patients. (I will guess that your fees to exploit these broken people are not roleplayed though)

If you mean that literally, a script in fact can very selectively detect avatars sitting on your furniture (as opposed to just standing around), and then eject* them from the parcel. Of course furniture itself has always been able to tell who's sitting on it, but instead of needing a script in each item of furniture, a centralized script can monitor llGetAgentList then use llGetObjectDetails to not only find whether they're located in the area of interest (OBJECT_POS) but also what if anything they're sitting on (OBJECT_ROOT) -- and then maybe llGetObjectDetails(OBJECT_OWNER) of that object to make sure it's your furniture they're jumping on.
