How do I fix this problem?

Contact the guy that made the avatar. You can look at the PROPERTIES of the item in inventory to get the creators name. Depending on your viewer you can click on the name and find that person's profile. Read the Profile to see if you need to email or IM or send a note card to that person. For a lot of us IM's fill up and we miss IM's arriving after the box is full.

Often when you see attachments weirdly rendering you can fix them by logging off and back on.
