I believe this happens, but probably it's rather awkward, so understandably people might assume it was intentional. Here is a post I made some time ago:

I have actually lost friendship with my alt --- and obviously I didn't do that, so yes it can happen. DID it happen? That is a different question of course.

If you find out how it happened, tell Maddy how it was done. She'll be really grateful

you have more than one account, have you had the same friend vanish from BOTH accounts at the same time from a SL glitch. I mean people say its rare, so if its rare a friend disappearing from one account how about from 2 at the same time?

Why don't  you look your friend up and ask them if they still see you on their list(s).  At least it'll start a conversation.  More direct, eh?

Lol no. If your friend is deleted from BOTH of your accounts and you didn't do it, then they've deleted you.
