It's probably quite simple, but confusing. Many items are boxed for sale, especially if they have several pieces. The blob is a little stranger, but I suspect that it's part of your avatar, meant to be attached with the rest of it. There ought to be instructions in the folder that the items came in. If not, you should contact the merchant who sold it and ask for help.
Of all the things that you have to face as a newcomer in SL, choosing and adapting an avatar is one of the most confusing, especially if you dive in and buy a custom avatar instead of starting with something simpler, like one of the starter avatars that you get by clicking on the avatar picker in your viewer. I suggest doing some reading before you go much farther.
