A region or estate ban should prevent the avatar from entering at all. The Wiki suggests that the ban extends to 5000 metres above ground level.   However, if the banned avatar can fly up to >5000 metres and then overfly the region from which he's banned, he still can't descend to a lower altitude.   

My understanding -- which is based only on what people have told me, since I would not use an unauthorised viewer -- is that some griefer viewers allow the user to circumvent region and estate bans.
