Rhonda has told you how to release your land.  That's easy.  Don't expect to get your money back, though.  The TOS makes that part clear: "4.4 Linden Lab does not accept returns or provide refunds of any amounts paid for products or services purchased from Linden Lab."

You will remain a Premium member until the end of the period that you have paid for, however, so you will continue to get your weekly L$300 stipend and will have access to Premium sandboxes, Live Chat, and all categories of support cases. Your membership status will show as "Premium cancellation pending".
