Any type of "pregnancy" is roleplay, assisted with huds, scripts and objects. And not few systems only simulate the pregnancy phase and then leave it to you to buy a baby* or not.

 

 

*Babies are also just scripted objects. The "better" ones are compareable to the virtual pets some people buy and care for.
