In days of yore all kinds of weird and wonderful things used to happen similar and dissimilar to your own experience. Some of us have logged in and found ourselves wearing two outfits simultaneously, half our hair has been missing, or we had a shoe up our bottoms. As you got logged out, and had to relog, and found your avatar had been taken back to a previous version, it does suggest that possibly Linden Lab were doing something with the servers, your sim got restarted. Checking grid status pages, and there has been some rolling maintenance (usually happens on a Tuesday each week, but it rolled on Wednesday this week).
