Why is it that the categories of Avatar Appearance never show up?  Can someone fix this?

What sort of categories?  When you right-click on your av and select Appearance, you get two choices:  Edit Shape or Edit Outfit.  Whichever one your chose has a full menu of choices.

You will not be able to edit outfits or body shapes that are no-Mod.
